I think a lot of it has to do with wearing under wear shorts that compress the balls and sack up close to the body, not letting the testicles hang loosely. Since I quit wearing under wear some 20 years ago, my balls and sack hang much lower, and in conjunction with stretching the balls with rings and ball stretchers help also.
